Appointment Booking

Upon Arrival:

1. Provide your contact number and show your Identification Card to our security.
2. Every visitor will be issued with a visitor tag. Please wear it at all times while in SAS.

Please Take Note:

1. Visitation is strictly by appointment only.
2. If there is more than one visitor, you will need to add them as visitors in your registration.
   There is no limit to the number of visitors
3. Children age 6 years and below need not register.
4. Mask-wearing is required in the Nursing Home premise.
5. If you are unwell or in close contact with a COVID-19 case, please refrain from visiting.
